CHOICES Intake & Screening Specialist

Duties/Responsibilities:

• Conduct comprehensive telephone screenings from program applicants.
• Provide information and assistance to potential clients.
• Make referrals to Long Term Care CHOICES Assessment Coordinators for more in depth assessments.

Qualifications/Requirements:

• Graduation from an accredited two year college with an A.A. Degree in Nursing, Social Services or related field;
• Considerable experience working for a social service agency or department; 
• Good interviewing and assessment skills;
• Good organizational, interpersonal, and decision making skills; or
• Any equivalent combination of education and/or experience.
• Valid TN driver license.
