Healthy Steps Specialist

The mission of Bronx Health Collective (BHC) at Montefiore Medical Center is to provide comprehensive, compassionate, family-centered health care to New York City’s most medically underserved children and their families, both homeless and domiciled. BHC is a Federally Qualified Health Center (FQHC) that serves nearly 9,000 patients each year through a network of health care centers and a fleet of mobile health care clinics serving residents of NYC shelter system. BHC is also recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home (PCMH) with Behavioral Health Integration Distinction (BHID).  Residents of the South Bronx communities served by BHC live in some of the poorest congressional districts in the United States. They experience higher rates of asthma, obesity, diabetes, infant mortality, premature mortality, and psychological distress than other communities in NYC. In response, BHC has developed, and continues to develop, creative programming to meet the needs of our communities through Special Initiatives. One of the Special Initiatives of BHC is our Healthy Steps program, which provides additional education and support to families with young children to optimize child health and well-being. 

We are looking for a Healthy Steps Specialist, who will join the pediatric primary care team at Bronx Health Collective to provide interventions, referrals, and follow up of families with patients ages 0 to 3. The Healthy Steps Specialist builds strong relationships with families and providers to increase the efficiency of the medical system and support team-based comprehensive pediatric care.

Some of the specific duties of the Healthy Steps Specialist is to:

  • Conduct team-based well child visits before, during, or following the primary care provider. Visits focus on monitoring development, social-emotional functioning, and relational health, and providing anticipatory guidance.
  • Provides consultations on a short-term basis for needs regarding behavioral concerns such as sleep, positive parenting, picky eating, etc. 
  • Provides referrals and tracks follow up as appropriate to help families make successful connections to key resources within the community.

Position Requirements

  • Masters degree in social work
  • Eligible for licensure as a LMSW or LCSW
  • Preferred: 1-2 years of experience in early childhood assessment and intervention
  • Preferred: Spanish fluency

Additional Information

  • This is a full-time position and will report in-person at our site/s in New York City
  • Salary: $31 – 47 /hour

Knowledge preferred

Experience and knowledge about early childhood growth and development, parent-child relational health, infant and early childhood mental health, and family systems. Strong understanding of services offered by and referral processes for early intervention, CPSE, OPWDD, and related programs. Comfortable utilizing therapeutic treatments and interventions.
